Move the documentation to `doc/plugin/GhostInTheFirmware.md`, sources under
`src/kaleidoscope/plugin/` (appropriately namespaced). This is in preparation of
merging plugins into a single monorepo.

While the original plugin was written independently, significant developments
were made while working for Keyboard.io. As such, I feel it is appropriate to
assign copyright to the company.

`USE_PLUGINS` and `loop_hook_use` are deprecated, use `Kaleidoscope.use` and
`Kaleidoscope.useLoopHook` instead.
